2018 Registration/Certification Information Sheet

 *** REGISTRATION AND CERTIFICATION ARE TWO SEPARATE BUT RELATED AND REQUIRED PROCESSES ***
REGISTRATION: The 2018 Southfield Ravens’ Registration DEADLINE is May 30, 2018  Registration involves completion and submission of all documentation included in registration packet and payment of all applicable registration fees.
All Ravens players are required to complete registration prior to being certified by DPAL, placed on a team, or given equipment/uniforms.

Registration Checklist

#1 Ravens Registration Form
#2 Detroit PAL Contract
#3 Michigan High School Athletic Association, Inc. Form (Physical):
– COMPLETED & SIGNED by a medical professional
– Dated after January 1, 2018
3. STAMP with the medical professional’s name, title and address.  (2 Copies) All participants must submit the Medical Clearance Form before participating in any PAL programs, including pre-season practices and conditioning camps. NOTE:  After the season begins, the Physical Form will become void in the event of an injury, accident, or illness that is attended to by a licensed medical professional. The Resume Participation Medical Clearance will be required in order for the participant to resume active participation.
#4 Birth Certificate:
(2 Copies of Original) A COPY of the participant’s ORIGINAL Birth Certificate MUST be submitted by May 30, 2018.  An ORIGINAL must be presented at face-to-face league certification – even if the participant played in a previous year. NO HOSPITAL CERTIFICATES WILL BE ACCEPTED.  NOTE:  We destroy all confidential information seven years after the season.

2018 REGISTRATION FEES

NON-REFUNDABLE DEPOSIT required for all participants in order to reserve a spot.

Fees Per Participant:

Registration Fee and Cheer Items (Spirit Pak) Total: $400

  • 1st Payment Due April 27, 2018 Without Fundraising: $100
  • 2nd Payment Due May 25, 2018 Without Fundraising: $100
  • 3rd Payment Due June 15, 2018 Without Fundraising: $100
  • 4th and Final Payment Due July 13, 2018: $100

*There are additional costs that will be incurred throughout the season, as these fees DO NOT cover the purchase of mandatory items that are not listed above, travel or transportation costs to out of town venues, special events and other specific items not defined. Participants on the travel/competition teams are chosen on a try-out basis.*

ALL registration fees cover operation expenses, insurance, league fees and equipment & uniform rental specific to each group and other items incidental to participation in The Ravens Athletics. All issued uniforms are the property of the The Ravens and must be returned at the conclusion of the participant’s season. Spirit packs are not included in the regstration fees.

By signing below I am agreeing to allow The Ravens to use my child's photograph, video & audio recordings in any future publications, web pages & other promotional materials produced, and that there will be no compensation to me for this use. I understand that my child is not guaranteed a roster spot, and equipment and uniforms shall not be issued, until 100% of the registration fee has been paid and all required documentation has been submitted. I also understand that The Ravens Football & Cheerleading Association is under no obligation to contact me with payment reminder(s).
